Remember to bring: a list of your medications, name and zip code of your pharmacy, your current Medicare Part D card if you have a plan, and your Medicare card.
Medicare Prescription Drug Coverage (Part D) is partially subsidized by the Federal Government. To be eligible you must be entitled to benefits under Medicare Part A and/or enrolled under Part B.
